Waiting time
After registration, an initial appointment is scheduled by mutual agreement as soon as possible, usually within one month.
Temporary absence
In case of temporary absence due to illness, clients are informed of this and what is appropriate is discussed.
Reimbursement
Reimbursement by the health insurer depends on your policy and the conditions of your insurer and may change from year to year. Please check with your health insurer. No rights can be derived from any (partial) reimbursement.
Rates
Rates are discussed with the client in advance and are available on request, are visible in the practice.
Important information
The treatments fall under complementary care and do not replace regular medical care. In case of (persistent) complaints, it is always advised to consult a doctor or medical specialist. No medical diagnoses are made and no cure guarantees are given. The practice does not offer crisis care. In acute situations or in case of psychological distress, it is advised to contact the general practitioner or GP post. Counselling may include conversations, insightful counselling and complementary forms of work, tailored to the request for help.
Practice conditions & treatment arrangements
1. General
These practice conditions apply to all counselling, treatments and appointments between Madeleine Meuwessen and the client.
2. Treatment agreement (WGBO)
At the start of counselling, a treatment agreement is concluded in accordance with the WGBO. This is explained during the intake interview.
3. Appointments and cancellation
Appointments can be cancelled or rescheduled free of charge until no later than 24 hours before the start, exclusively by e-mail. If cancelled within 24 hours or in case of no-show, the consultation may be charged.
4. Payment
Payment is made immediately after the consultation by pin, or by invoice, payable within 3 days. If payment is not made, the practice reserves the right to suspend further appointments.
5. Reimbursement by health insurer
Reimbursement by the health insurer depends on your policy and the conditions of your insurer and may vary from year to year. The client is responsible for inquiring with the health insurer. No rights can be derived from any (partial) reimbursement.
6. Liability
Counselling is carried out with the utmost care. Madeleine Meuwessen is not liable for damage resulting from incorrect or incomplete information provided by the client, nor for damage resulting from decisions made by the client himself as a result of the counselling. No medical diagnoses are made and no cure guarantees can be given.
7. Privacy and client file
Personal and client data are treated carefully and confidentially and recorded in a client file in accordance with the AVG and WGBO. The full privacy policy is included below and forms an integral part of these practice conditions.
8. Complaints procedure (Wkkgz)
If you are not satisfied with the treatment or service, preferably discuss this with me first. If we do not come to an agreement together, you can use the independent complaints officer through the NWP. If necessary, the complaint can be submitted to the recognised arbitration body (Camcoöp). In serious complaints, referral to the disciplinary tribunal may take place; in exceptional cases, the Healthcare and Youth Inspectorate (IGJ) may be involved.
9. Applicable law
These practice conditions are governed by Dutch law.

Where applicable, this is done with your (explicit) consent or on the basis of a legal exception within the AVG for healthcare provision.

Madeleine Meuwessen does not intend to process personal data of persons under the age of 16 without the consent of parent(s) or guardian(s). If you believe that data of a minor has nevertheless been collected without consent, please contact us at [email protected]. Then we will delete this information.
WHEN DOES MADELEINE MEUWESSEN PROCESS YOUR PERSONAL DATA?

If you contact us by phone, e-mail, form or letter, we process data to be able to help you and to handle our contact properly.

COOKIES WE USE
Madeleine Meuwessen uses functional cookies and, where applicable, cookies that require consent. A cookie is a small text file that is stored on your computer, tablet or phone the first time you visit a website. If permission is required, we will ask this during your first visit. You can disable cookies via your browser settings.
TRANSFER TO PARTNERS (PROCESSORS)
To perform its services, Madeleine Meuwessen may use suppliers, such as hosting, e-mail, newsletter software (if applicable) and administration software. Appropriate arrangements (such as a processing agreement) are made with parties who process personal data on behalf of Madeleine Meuwessen to protect your data. Madeleine Meuwessen only cooperates with parties that can comply with the AVG. If data is processed outside the European Economic Area, this only happens if there are appropriate safeguards (e.g. via standard contractual clauses).
TRANSMISSION TO THIRD PARTIES
In principle, your data will not be sold or shared with third parties, except if this is necessary for the performance of the agreement, to comply with a legal obligation, or if you explicitly consent.
SECURITY OF YOUR DATA

HOW LONG WILL YOUR DATA BE KEPT?
Madeleine Meuwessen applies different retention periods:
Correspondence (e-mail/messages):
In principle, a maximum of 5 years, unless longer retention is necessary for the performance of an agreement, the settlement of a complaint/dispute, or to comply with legal obligations.
Invoices and administration:
7 years (legal tax retention period).
Client file (if applicable):
Health and treatment data are kept as long as this is necessary for proper care and in accordance with the legal retention periods usual in the Netherlands for health care records (usually 20 years), unless you request destruction earlier and there is no legal or compelling reason to refuse this.
YOUR RIGHTS
You have the right to access, correct, delete, request restriction of processing, object and request data portability of your personal data. If the processing is based on consent, you can always withdraw that consent. You can send a request to [email protected]. To ensure that the request was made by you, you may be asked for a copy of your identity document.
In that copy, make your passport photo, MRZ (machine readable zone), document number and Citizen Service Number (BSN) unreadable. You also have the right to file a complaint with the Personal Data Authority.
CAMERABLES
There is no camera surveillance in the practice space. Any camera security around the home is solely for the purpose of securing the residential home and does not target clients or the practice space.
REPORTING A DATA BREACH
If, despite all measures, a data breach does occur, Madeleine Meuwessen will act in accordance with the data breach notification obligation. Should you discover a possible data breach yourself, please report this as soon as possible via [email protected].
LIABILITY
This privacy statement only covers the personal data that Madeleine Meuwessen processes within its own services. Madeleine Meuwessen is not responsible for the content or operation of third-party websites or services referred to.
